Acknowledgement of Risk andWaiver of Liability

I understand that I will be participating in an exercise and wellbeing program through Pilates Success, Sandra Leigh Training and / or Healthy Office Bodies that will require physical exertion.

I understand that it is my responsibility consult with the physician prior to and regarding my participating in any fitness program.

I represent and warrant that I’m physically fit and have no medical condition that would prevent my full participation in this program and I take full responsibility for any that I have failed to advise.

Inconsideration of being permitted to participate in any group movement/exercise class I agree to assume full responsibility for any risks, injuries and damages known or unknown which I might incur as a result of participating in the program.

If I observe any sign of discomfort pain or hazard during my participation, I will remove myself from participation.

I understand that, by reading this statement, I am agreeing to not hold Pilates Success, Sandra Leigh Training and / or Healthy Workplaces or any of its employees, owners, agents, or insurers responsible for any bodily injury or property damage that may suffer as a result of my participation in a fitness program through Pilates Success, Sandra Leigh Training whether at Pilates Success, at home, remote or elsewhere.

Cancellation and Payment Agreement- All services

Life gets in the way sometimes and with that in mind, Pilates Success, Sandra Leigh Training will not charge for a cancellation if appropriate notice is given.

If an appointment is cancelled with less than 24 hours’ notice, or if you do not attend a scheduled appointment, full fees will apply. You will be charged AND / OR credit deducted from your prepaid package.

If you have not prepaid, please make payment at your next scheduled appointment. We apply a ‘no refund policy ‘ as per Australian Consumer law

Media Statement

I hereby grant all media rights and interests to Pilates Success and/or Sandra Leigh Training. Media file types such as – Zoom recordings, audio, voice, all statements, photographic images, video recordings that are made by Pilates Success and/or Sandra Leigh Training

General statement of program (Chi Flow Sunday, Radiant Women, Harmony in Motion)

I understand that theses programs includes exercises of a beginner, mindful nature. This means it will be slow and gentle designed for safety and ease of practise. Almost all of the movement will be in a standing position, with a chair used on occasion.

Remote Exercising – Description of potential risks

I understand that I’m exercising in a remote environment without direct supervision, and take full responsibility for my body during the movement components of this program.


I understand that all exercise in any context can lead to abnormal blood pressure levels, musculoskeletal strains, pain and injury if adequate warm up, gradual progression and safety procedures are not followed.


Floor space – Only a small about of standing space is required preferably and approximately 1.2. metres beyond the physical body clear of any furniture.


All floor space must be cleared of any trip hazards and I take full responsibility for my personal remote environment.


I will limit distraction by way of other devices by turning off all other devices whilst participating in the exercise and wellbeing program.


I will participate in a quiet environment away from distractions such as other people and pets.


I will ensure that the room temperature is comfortable at approximately a recommended temperature of 22 – 24 degrees.


I will ensure that I am well hydrated by having a drink of water prior to starting the exercise and then ongoing by having a water bottle nearby if further hydration is needed.


By reading this, I am stating that I have read the above release and waive of liability and understand its contents. I voluntarily agree to the terms and conditions stated above.
